Welcome!

Whether you are expecting your first child, transferring from another state or just looking for a  change, we hope you will take a few minutes to learn why DLC Pediatrics is special.

At DLC Pediatrics, each patient is greeted with a friendly smile by our trained professional staff. Our computarized facility makes our office more efficient, thus reducing your wait time in the lobby and allowing the doctor more time with your child . Every exam room has an exciting theme that will capture your child’s imagination  at first glance. Flat Panel Displays in each exam room. These  displays are used to comfort your child by providing entertainment and patient education.

Ready to Make the Change?

Once you decide your child will become a DLC Pedicatrics patient, please go to the FORMS tab and review, and complete all New Patient Forms prior to your appointment to reduce your wait time.

Also, bring a picture ID from one of the parents or guardians and the insurance card. For newborns, all of these apply as well but also we will be needing the paperwork from the hospital.

If you want us to request medical records from another clinic, please bring that information as well.

Policy Highlights

Appointment: Please arrive before your scheduled appointment to allow time to complete any required registration forms. Bring your insurance card to every appointment. We will verify your insurance and personal information at each visit. You will be allowed a 15 minutes grace period for being late after which you will be given the opportunity to reschedule or wait for cancellations.

Cancelled Appointment: If you are unable to keep your scheduled appointment, please call our office within 24 hours to reschedule. Doing so enables us to use your appointment slot for another patient. This policy benefits you and others. Unfortunately, if you fail to make an appointment, without calling to cancel, you will be considered a “No Show”.

No show appointments will be charged a $20 fee, this can be avoided with a 24 hours to an hour notice.

Medical Records: We will provide a copy of your medical records upon request. You will need to sign a letter of release prior to pick-up. We will provide information within 15 days from receipt of request. If you wish for your records to be mailed, there will be an associated fee to cover the mailing costs. Records that are requested for your personal files will require a $6.50 fee.

Copy of Immunizations: There is no charge for the first copy of the year any additional copies, there will be a charge of $5.00 per copy.

Medications: Your prescriptions will be written at the time of your office visit. Refill request may be through your pharmacy or by calling our office. Please be aware that for certain illnesses your refill may require an appointment. We will not prescribe any medications bases on telephone consult.
